PATIENT INFORMATION

• Please be prepared to show your current State or Federal issued Photo ID, Driver's License or Passport at the time of visit.
• Bring current insurance card(s) and present upon checking in.
• ​Co-pays are due at the time of service.
• To give permission for our office to speak with persons other than yourself regarding your healthcare, please fill out the Permission to Relay Information form. (Also part of the New Patient Packet.)
• Please update your Health History Form or your Annual Wellness Form (65+) yearly.
• If you require that our records be sent to another physician, please fill out an Authorization to Release Records for each provider you want records sent to.
• To see how your health information is used you can read our Notice Of Privacy Practices.  You may print or save a copy for your records.
• All patients must sign an Acknowledgement of Receipt of Notice of Privacy Practices on file, agreeing that you have either received or read our Notice of Privacy Practices.

• You can save time at your first appointment by filling out and printing the New Patient Packet prior to your arrival.
• Please fill out a Request for Records for each doctor, hospital or facility  you are requesting records from.
• You are considered a New Patient if it has been 3 or more years since you were last seen in our office.
• Our providers do not perform physical exams or well woman exams on New Patients. All patients come in to Establish first. (*certain exceptions may apply and will be decided by the provider on an individual basis.)

• HMO: We only participate with Greater Newport Physicians (GNP)
  
• PPO: We are no longer accepting new PPO patients. Existing patients that transition to a PPO plan that we are contracted with will be accepted.* 

• Medicare: We are no longer accepting new Medicare patients. Existing patients that transition to Medicare will be accepted.*
